According to the government regulations, we offer you to scan LeaveHomeSafe app and vaccination record.

ワクチン接種証明は原則としてQRコードでの読みとりを実施しておりますが、海外で受けた接種記録をQRコード化されていないお客様につきましては接種証明書の提示と政府規定の用紙に入店記録をご自身で記入頂きます。

Although we carry out scan by QR code in general, if you still do not make vaccination record to QR code, we ask you would fill in the specific paper by yourself .

4月30日から5月30日までは、

From April 30th to May 30th,

・18歳以上のお客様は有効なワクチン接種を2回完了されている方のみ

above 18 years old : must have done effective 2nd dozen

・12歳から17歳のお客様は1回目の接種から6カ月未満の方は1回接種、6カ月以上経過されている方は2回接種のみ

12 years old to 17 years old : only take 1st dozen if it still does not pass 6 month from first dozen or must 2nd dozen if it passed 6 month from last dozen
